A factory ships 12,000 blouses to a premium brand. During the final inspection the buyer finds that the buttons are two millimetres larger than the approved trim card. The whole shipment is placed on hold.

The investigation shows that the merchandiser sent a revised button specification by email two days before cutting, but the email was never added to the production file. Cutting and sewing continued with the old trim card. There is no written approval of the change, no updated sample and no record of who received the email.

From that day the factory introduces a simple rule: no specification change is valid without a signed revision sheet, a physical sample attached, and a confirmation reply from the factory manager. Rework costs drop and the next audit scores improve because every change is traceable.
