A factory holds a 9:00 a.m. daily production review. The line supervisor reports that line three is at 82% of target because fabric for the next style has not arrived from the warehouse. The production manager thanks the supervisor and moves to the next line without assigning anyone to solve the problem.

By noon, three more lines are waiting for the same fabric. The PP meeting at 2:00 p.m. becomes a crisis call. The shipment is delayed by two days and the buyer is informed late.

A better daily review would have captured the decision immediately: warehouse manager to confirm fabric location by 10:30 a.m., floor-in-charge to re-sequence lines by 11:00 a.m., and production manager to notify the buyer by 12:00 p.m. if the delay exceeds four hours.
